To all Esposito Nursery Inc. employees,

Please be advised of an update regarding the delivery of your annual Form 1095-C, Employer-Provided Health Insurance Offer and Coverage.

Esposito Nursery Inc. is no longer required to mail a copy of Form 1095-C to every employee. Instead, the Form 1095-C is now available to you upon request.

If you require a copy of your Form 1095-C, please visit the Human Resources department during regular business hours to request and receive your form.

This notice is posted in compliance with the new regulations concerning Form 1095-C distribution.
